A lot seems to have changed with the latest generation of tensors. They main couple of things are thinner topsheet and thicker sponge. Previously max was 2.2 or 2.1mm and the next level of thickness below these was 2.0mm or 1.9mm, depending on the brand.

Now with the latest generation sometimes being quoted as Max +, their next thickness down can be 2.1mm.

Donic Bluestorm - Max +, 2.1mm, 1.9mm
Andro Rasanter - Max, 2.0mm, 1.7mm (Andro previously Max, 1.9mm, 1.7mm)
Tibhar Aurus - Max, 2.1mm, 1.9mm, 1.7mm

I'm used to buying the level beneath max in all rubbers to give a little more control and depending on the brand it might be 2.0mm in say Baracuda or 1.9mm in Tibhar Evolution series.

Given the change in thicknesses, using the same theory, should I now be ordering the same thickness as previous ie 2.0mm or 1.9mm or should I be ordering one level lower than max which could be 2.1mm?

Also in regard to sponge hardness I've read that the thinner topsheet affects how the sponge plays. In this case if you previously using a 45 degree sponge, should you now order the same as what you previously liked or maybe a bit harder or softer to compensate for the thinner topsheet?

If they went to the trouble of designing a thinner topsheet to accommodate a thicker sponge then it makes the most sense to use the max thickness. Otherwise, you aren't really getting the most out of the design. IMHO

(Just get max on the forehand side. It's a powerful stroke and you should be able to get the most out of the sponge. )
